Your Rights

You have the right to: request a copy of your health information, request corrections, request restrictions on how your information is used, request an accounting of disclosures, receive a paper copy of this notice, and file a complaint if you believe your rights have been violated.

How We May Use or Disclose Your Information

Standard uses include: Treatment (sharing with other providers involved in your care), Payment (submitting claims to insurance), Healthcare Operations (quality review, training), and Required By Law (mandatory reporting situations including child abuse, elder abuse, and imminent risk of harm to self or others).

Limits of Confidentiality

As noted in our FAQ and intake paperwork, there are specific legal exceptions to confidentiality: if a client states they will hurt themselves or someone else; if there is a concern about possible child abuse or neglect; or if there is a concern about abuse, neglect, or financial exploitation of an elder or disabled adult.

Our Responsibilities

We are required to maintain the privacy of your health information, provide you with this notice, follow the terms of this notice, and notify you in the event of a breach of your unsecured health information.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Aligned Associates PLLC at alignedassociates@gmail.com, or with the Secretary of the U.S. Department of Health and Human Services privacy officer at 129 Pleasant Street, Concord, NH 03301, or DHHSPrivacyOfficer@dhhs.nh.gov. We will not retaliate against you for filing a complaint.

You may also write to the federal government by contacting the Office for Civil Rights at the John F. Kennedy Building, Room 1875, Boston, MA 02203 or ocrmail@hhs.gov. You will not be penalized or retaliated against for filing a complaint.

Tebra — HIPAA-Compliant Platform

All telehealth sessions are conducted through Tebra, a HIPAA-compliant electronic health record and video platform. Session data is encrypted and stored in compliance with HIPAA requirements.
